DALLAS—Students in the Dallas Independent School District will be required to complete a series of vaccinations designed to prevent childhood diseases prior to the start of classes Aug. 18, in order to comply with guidelines established by the Texas Department of Health.

All immunization records must be validated by physician's signature or health clinic stamp. The month, day and year must be indicated on the record for each new immunization. The immunization requirements for the 2003–2004 school year include:

The second dose is due at age four, and must be received at least 30 days after the first dose. A second dose of rubeola is due by the 12th birthday for students born prior to Sept. 2, 1991. Serologic confirmation of rubeola immunity is acceptable in lieu of immunization dates.
