Form available in student enrollment packet; parents have 15 days to sign and return to school
DALLAS—Dallas ISD parents may choose to keep their student's directory information from being released by signing privacy forms which can be found in student enrollment packets distributed by each school.
Information such as the student's name, date and place of birth, address, phone number, grade level, and most recent school attended may be withheld if parents sign and return the form to the school within 15 school days from the time the enrollment packet is distributed.
To be in compliance with the No Child Left Behind Act of 2001, Dallas ISD releases the names of secondary school students, their addresses, and telephone numbers to military recruiters and institutions of higher education, upon request, unless the parent or guardian directs Dallas ISD not to release this information without prior written consent.
Students are required to have a new form that is signed by a parent or guardian and submitted at the beginning of each school year to ensure that their directory information is not released to anyone, including non-Dallas ISD affiliated organizations that request it.
